My shoulders tend to get very tight and tense throughout the day. When it gets really rough, I put this across the back of my shoulders, and it seems to help. The pack is a little bulky though so probably not possible to wear if you're going to work (it says not to fall asleep with it on) or trying to be subtle about having it on under your clothes. However it's good for when you're at home and trying to relax!

Mom's neck hurt for a long time, nothing helped at all, no ointment, no pills. Found this patch in old stocks. This thing helped in literally two uses. then it turned out that it was impossible to buy it from us. It's good that there is Iherb))
